Athlone FRC Social Prescribing Service:
• Focuses on “what matters to the person” rather than “what is the matter with the person”
• The Link Worker works in collaboration with a person over a period of time using a personalised coaching and co-production approach
• The aim is to connect people with sources of support within local community with the purpose of improving health & wellbeing
What is Social Prescribing?
Social Prescribing is part of a holistic approach to addressing the health and wellbeing needs of individuals by supporting them to access a range of services and groups based on what is important to them, their interests, and their personal goals. This can include supported access to both clinical and non-clinical services and groups.
Some examples of these are women’s groups, men’s shed projects, fitness programmes, creative arts, gardening, smoking cessation, counselling, parenting programmes etc. It really is up to the individual. We will work from where the person is at and what they want to do or achieve. The Social Prescriber Link Worker is there to work in partnership with the individual to create a personal plan and to support them to fulfil this plan.

Joanne Naughton holds a BA (Hons) in Applied Social Studies and a MA in Child and Youth Studies. She has over 20years experience working within the Youth and Community Sector.
